請假條格式：
標題：居中寫《請假條》，用來表明此文是用來請假的。
稱呼：平時怎麼稱呼，這裏就怎麼寫，後面加冒號。這個地方往往是領導、相關負責人等。
正文：請假理由。直接寫請假的原因，起止時間。如果有什麼事要交代，也可以在寫完請假原因之後接着寫要交代的事情。正文寫完後要加祝頌語，祝福對方，這個是所有公文裏表示對對方的友好。
署名：寫在正文下右下方第三行處。如果是學生向老師請假，在名字前面加上“學生”二字。
日期：寫在署名下面，另起一行。也可以不寫年，最好在月日下面寫上上午或者下午，也可以寫上時分，必要時加上地點。
寫作要求：請假條最重要的內容是寫明“請假原因和日期”，一定要寫明確，不能只寫“因有事請假”。
